Spain’s healthcare system has long had a solid reputation among foreigners living here, but the latest global data gives that reputation some proper context.

In Numbeo’s 2026 Health Care Index, Spain ranks 10th in the world, placing it firmly among the highest-performing systems globally.

Spain compared with other top healthcare systems

More than half of the top 25 healthcare systems are European, with Nordic and Western European countries consistently scoring highly due to broad access and lower financial barriers.

Spain achieves the same index score as Denmark and France while spending significantly less per person than Denmark.

Healthcare spending: Spain vs higher-spending Countries

One of the clearest patterns in the 2026 ranking is that higher spending does not automatically lead to better performance.

Despite spending roughly half as much as some northern European countries, Spain remains in the top 10. The broader conclusion highlighted in the ranking is that systems prioritising universal access, preventative care and efficiency tend to perform strongly.

Spain vs the United States

For Americans weighing up a move, the contrast is particularly sharp. The United States spends $13.500 per capita, more per person than any country in the ranking. The country places 40th overall.

Spain ranks 30 places higher while spending less than a quarter per capita, at just $3,100 per capita in Spain.
